MAIL-BOX SIGN-UP

To sign up for a mailbox, please fill out the form below. Please also download USPS Form 1583, fill it out, have it notarized and send the original to us. Our mailboxes are $20 per month, and we bill 3 months at a time if on autopay, or 6 months at a time if on "pay as you go" status. You can have your mailbox on autopay with either a paypal account or by giving us a credit card on file, and your mailbox will be automatically charged to you on a quarterly basis ($60 per quarter). The preferred method is paypal, as it gives you secured, complete control over the account without giving us any of your credit card info, is completely automated for both you and our store, and provides notification and history of all payments.

All mailboxes are pro-rated to the closest quarter so everyone is on the same billing cycle. The quarterly due dates are January 1, April 1, July 1 and October 1 of each year.

There is a $10 set up fee and a $5 key deposit per key and/or 24hr Key FOB. The 24hr Key FOB provides access to your mailbox 24hrs a day.

Your initial fee will be calculated as the pro-rated amount until the next billing date plus the set up fee and key deposits. If you select autopay, then your pro-rated amount will be calculated to the next quarterly due date. If you select pay as you go, your pro-rated amount will be calculated to the next quarterly due date PLUS the following quarterly fee of $60.

Terms of Agreement

USPS Form 1583

Both parties shall be governed by these terms to which each party agrees:

1. Applicant agrees no mail will be released until USPS form 1583 along with a photocopy of 2 acceptable Id's is notarized and returned to Eastlake Mail office. Eastlake Mail will provide form 1583 and instructions at time of signup and upon request at anytime.  (A copy of USPS Form 1583 can be found in this .pdf document, or can be downloaded from USPS at http://www.usps.com/forms/_pdf/ps1583.pdf)
			
2. By completing this form and USPS Form 1583, a copy of which will be made available to the United States Postal Service, applicant appoints Eastlake Mail as agent for the receipt for a period not to exceed that for which rent has been paid in advance. Applicant will pick up mail at least once each month or make other suitable arrangements, in advance, with Eastlake Mail. Eastlake Mail will provide a mailbox key to applicant who may obtain his mail during the business hours posted by Eastlake Mail. Should applicant appoint another person or organization, Eastlake Mail shall assume that possession of a key is evidence of authority to collect mail.
			
3. The key (and/or 24hr Key FOB for 24hr access) loaned to applicant shall require a refundable cash deposit, and remains the property of Eastlake Mail and shall not be duplicated or modified by applicant. Applicant understands that the relationship of the parties hereto is one of bailment and not landlord and tenant.
			
4. Once Eastlake Mail has placed applicant's mail in the assigned mailbox, the mail shall be deemed to have been delivered, and Eastlake Mail shall not be responsible for loss, theft or damage. Eastlake Mail is not engaged in the delivery of mail and cannot be responsible for failure of the United States Postal Service to deliver mail or to deliver it in a timely fashion or undamaged condition.
			
5. Applicant agrees to use services in accordance with Eastlake Mail rules and in compliance with all U.S. Postal regulations, as well as local, state and federal statutes and regulations. Failure to do so may result in cancellation of service without notice, refund or mail forwarding.
			
6. Information provided by applicant will be kept confidential and will not knowingly be disclosed without applicant's prior consent, except for law enforcement or postal operation purposes, in which case Eastlake Mail intends to cooperate fully. Law enforcement is further clarified to include all city, county, state or federal agencies or their representatives.
			
7. Mail will be accepted for a standard-sized box no more than 125 pieces delivered per month, for up to five (5) persons or organizations and each must complete a USPS Form 1583 and provide photo identification. If applicant consistently receives substantially more mail than can be placed in a single mailbox, Eastlake Mail reserves the right to require applicant to rent a larger size box or one or more additional boxes. Charges for service are based upon average daily volume and activity. Special circumstances, e.g., high number of parcels, etc. may require assessment of additional fees. An unusually high volume of mail will result in either a higher fee being charged, or termination of the mail receiving service.  
			
8. Parcels are defined as any item for delivery that does not fit into the applicant's mailbox.  Applicant agrees that parcels delivered to this address for the applicant will be delivered by common carrier only, that no truck line deliveries will be made, that parcels will be retrieved within 48 hours after delivery, and that no hazardous or dangerous material will be delivered to applicant.
			
For any parcel that is not retrieved within 48 hours after delivery, applicant agrees to a $1 fee assessment per additional day beyond the first 48 hours.  For any parcel that is not retrieved within one week after delivery, applicant agrees that Eastlake Mail can, at its option, return the parcel to sender.  Applicant understands and acknowledges that it is not the responsibility of Eastlake Mail to notify applicant when parcels have been delivered for them, but rather that it is the applicant's responsibility to know when to expect parcels and to check regularly to make sure to pick them up in a timely manner.  Failure to adhere to any of these parcel delivery stipulations will result in termination of service.
			
9. Applicant agrees to protect, indemnify and hold harmless Eastlake Mail from and against any and all claims, demands and causes of action of any nature whatsoever relative to the use of Eastlake Mail facilities or services including without limitation, any demands, claims, and causes of action for personal injury or property damage arising from such use or possessions from:  (1) Failure of the U.S. Postal Service to deliver on time or otherwise any items (mail, parcels, etc.)  (2) Damage to or loss of mailbox contents by any cause whatsoever and (3) Any violation by the customer of applicable federal, state, or local laws.
			
10. Should Eastlake Mail commit or fail to commit any act that results in disruption of service and applicant thereby suffers a loss, Eastlake Mail's liability shall be limited to not more than the rental fees paid by applicant for service not yet received. Eastlake Mail shall not be liable for incidental or consequential damages.
			
CUSTOMER HEREIN AGREES THAT THE TOTAL LIABILITY OF EASTLAKE MAIL FOR ANY AND ALL CLAIMS IS LIMITED TO $50 REGARDLESS OF THE NATURE OF THEIR CLAIM.
			
11. Per USPS regulations, certified, registered, insured, or C.O.D. mail or parcels will be accepted by Eastlake Mail on the behalf of applicant. Full, advance payment of C.O.D. charges must be made to Eastlake Mail prior to acceptance of C.O.D. packages.
			
12. Eastlake Mail fees are due and payable in advance and notice thereof will be emailed and/or placed in applicant's mailbox. No other notice will be required. Failure to pay such fees when due may result in immediate disruption or cancellation of services. Eastlake Mail does not provide refunds in the event of cancellation by applicant.  Fees may be changed at any time at Eastlake Mail's discretion.  If fees are to be changed, Eastlake Mail will provide 30 days notice.	

13. Applicant shall use only the address designation "PMB" or "#" to designate their address. NO OTHER DESIGNATION IS VALID. Specifically excluded is the use of suite, apt., dept., or other designators. The U.S. Postal service may refuse to deliver any piece of mail that does not include the PMB or # sign designation. Applicant is responsible for notifying correspondents of the correct address.
			
Furthermore, all mail to be delivered to applicant explicitly MUST have the correct box number listed on the mail or applicant's mail is not guaranteed to be distributed into the applicant's mailbox, and will likely be returned to sender.  This is the sole responsibility of the applicant, and applicant recognizes that any mail without a box number or with the wrong box number will be treated as incorrectly addressed and acknowledges that it is not the responsibility of Eastlake Mail to research the correct address.  Any time mail is correctly distributed when addressed incorrectly is not to be construed as part of the normal obligations of Eastlake Mail.

14. Customer further agrees that Eastlake Mail may terminate or cancel this agreement for good cause at any time.  Good cause shall include, but is not limited to: 1) abandonment, 2) use for criminal, illegal, or illegitimate activity, 3) failure to pay moneys owed, 4) use of mailbox inconsistent with this agreement, including receipt of unreasonable volume of mail, and 5) nonperformance of the terms of this agreement.  Renewal for additional terms shall be at Eastlake Mail's sole discretion.
			
15. Upon termination of services by Eastlake Mail or failure to pay rent in advance by applicant, Eastlake Mail shall not make applicant's mail available without payment theretofore. A late fee of $5 will be charged if payment is not received within five days of due date.  Applicant understands that the United States Postal service will not forward or return mail without payment, and will not accept a Change of Address. At termination of service, applicant, if he wishes mail forwarded after that date, shall provide Eastlake Mail with a forwarding address and pay the required fees. In the event applicant fails to do this, Eastlake Mail shall refuse any further mail and, in the case of mail already received handle such mail in accordance with USPS DMM D042.2.6 regulations.  If necessary arrangements are not made for forwarding of mail when the agency's relationship has expired, is cancelled, or is terminated, the customer's mail may be returned to sender or destroyed.  Mailbox holder services may be terminated at the option of Eastlake Mail 30 days after the scheduled due date.
			
Termination Addendum
At termination of service, I hereby instruct Eastlake Mail as to do one of the following:
A:  Forward my mail to new address.
B:  Do not forward my mail. I understand that mail will not be forwarded and may be disposed of.

I have read and accept the terms of this mailbox agreement.

United States Postal regulations requires that each mailbox applicant fill out USPS Form 1583 and return it to us with notary signature, as well as a photocopy of two pieces of ID. You can download this form, fill it out and fax both the form and the two pieces of ID to us to initiate the mailbox rental, but we must receive the notarized original of USPS form 1583 before we can release mail to you.

USPS Form 1583 and instructions
